The Personalised Care roles in Primary Care are Care Co-Ordinator, Social Prescriber and Health and Wellbeing Coach.

The Personalised Care approach focuses on the question “what matters to you?” in contrast to the more traditional healthcare approach, “what’s the matter with you.” This stimulates a different and personal response. The increasingly complex needs and expectations of patients means that a one size fits all health care system is no longer viable.

In 2020, NHS England set up the Personalised Care Institute (PCI) and the below key aims as outlined on their website, are reproduced below. We recommend exploring the PCI website for information and guidance around the roles Care Co-Ordinator, Social Prescriber and Health and Wellbeing Coach.

The Institute was set up to:

  • set quality standards for training, focussing on 4 of the 6 components of the comprehensive model for personalised care
  • support the development of training programmes for the current workforce in order to help them to deliver personalised care to those quality standards
  • support the development of training programmes for the new roles identified within primary care networks as part of the NHS Long Term Plan
  • ensure that personalised care is represented in relevant undergraduate and post-graduate curriculums
